For Value-Based Care

Quality Dementia Care,

The Problem We're Solving

Dementia is one of the most underdiagnosed major conditions for seniors, and most patients and caregivers lack adequate care plans and support. This contributes directly to inaccurate risk scores, higher ED visits/hospitalizations, and increased total cost of care.

Kitwood offers a turnkey dementia care program that's fully staffed and reimbursable. Practices, ACOs, and health plans managing at-risk Medicare populations see:

  • Increased Diagnosis Accuracy
  • Reduced Total Cost of Care (TCOC)
  • Enhanced Patient Attribution
  • Improved Quality Metrics
  • Immediate Practice Profit

What Kitwood Provides

Population Analytics & Outreach

Kitwood makes proactive dementia care possible through population-based risk scoring. We use that scoring to prioritize a patient registry, then reach out directly to those patients for screening and neurocognitive assessments, rather than waiting for opportunistic screening through AWVs.

Fully Reimbursed

Requires no financial investment, and practices receive immediate profit through increased reimbursement, generating "good" fee-for-service revenue that is incremental and VBC-aligned.

Care management & Caregiver Support

Dementia is a progressive illness, and the support patients and caregivers need grows along with it. Kitwood stays in regular contact with both, offering caregiver education, help navigating community resources, and a direct line to us whenever a crisis hits.

Advance Care Planning & Crisis Management
Kitwood prepares patients and families for the healthcare decisions a medical crisis can force on them. Our team helps establish medical power of attorney and draft advance directives, then builds a crisis management plan specific to each patient. When crisis strikes, patients and caregivers can reach us anytime through our 24/7 call line.

Quality & Performance

Supporting Quality Performance

Undiagnosed dementia can erode performance on several quality and care measures. This shows up in practice as unexplained symptoms driving acute care utilization and poor medication adherence. Once a diagnosis is made and the patient receives active care and care management support, performance on these measures improves, including:

  • Hospital-Wide 30-day Readmission Rate
  • All-Cause Unplanned Admissions for Patients with Multiple Chronic Conditions
  • Emergency Department Visits (EDVs) that lead to a hospitalization
  • Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%)
  • Controlling High Blood Pressure
  • Falls: Screening for Future Fall Risk
  • Preventive Care and Screening: Screening for Depression and Follow-up Plan

Clinical Governance

Kitwood is physician-led. Dr. Stephen Bekanich, a palliative care physician and former CEO of Ascension Texas ACO, and Dr. Aaron Greenstein, a Harvard-trained, dual board-certified psychiatrist and geriatric psychiatrist, oversee clinical operations and standards across the model.
