Risk Scoring & Screening
Patients enter the cognitive care pathway through routine visits, clinician or care partner concern, or algorithm-identified risk factors. Kitwood supports reliable cognitive screening workflows aligned with Annual Wellness Visits and other appropriate care encounters.
Clinical Workup & Diagnosis
When screening or clinical concern points to further evaluation, we support a structured pathway from cognitive testing through diagnosis, without disrupting clinic flow.
Utilizing the best-in-class FDA-registered digital cognitive assessment from BrainCheck, we gather additional information to further pinpoint what is going on with the patient’s memory and thinking. From there, a cognition-focused E/M visit, along with labs, imaging, or specialist referrals as needed, helps determine whether an underlying medical cause is present.
We then apply current clinical practice guidelines to reach an accurate diagnosis through a comprehensive and clinically validated approach.
Personalized Dementia Care Plans
Assessment and diagnostic findings are translated into a structured, personalized care plan addressing medical, functional, safety, and care partner needs. Kitwood supports preparation, coordination, and documentation so care planning is complete, consistent, and actionable.
These care plan documents are practical next steps on what to do about the key areas of concern identified by the patients and care partners. The Kitwood Care Plan is a goal-directed roadmap, co-created with the patient and care partner, and anchored in the patient’s personal values, preferences, and clinical needs.
Ongoing Disease Management & Caregiver Support
Cognitive care does not end after diagnosis or care planning. This is where the impact begins. Kitwood provides structured longitudinal follow-up, reassessment coordination, and care partner education to ensure continuity as patient needs evolve.
All follow-up occurs within the primary care setting and is documented in the practice’s EHR instead of being farmed out to a three-party vendor.
